Case Management Nurse

Schedule: Hybrid – 3 days onsite / 2 days remote Start Date: September 28, 2026 Openings: 1 Important: This is a hybrid position and is not fully remote . Employees are required to work onsite 3 days per week, with Wednesday being a required onsite day . Position Overview We are seeking an experienced Registered Nurse (RN) / Nurse Case Manager to support employee health, wellness, medical leave, and absence management programs within an Employee Wellness Center. The Nurse Case Manager will coordinate employee medical leave and absence cases, including FMLA, maternity leave, short-term disability, long-term disability, catastrophic leave, and return-to-work activities . This position will work closely with employees, Human Resources, supervisors, union representatives, payroll teams, healthcare providers, and external partners to ensure employee health and leave cases are appropriately managed and documented. The ideal candidate will have strong FMLA and case management experience , excellent communication skills, and the ability to review medical documentation and coordinate complex employee health cases. Key Responsibilities Leave & Absence Management Manage and coordinate employee medical leave and absence cases. Administer and coordinate FMLA cases , including employee medical leave and qualifying family-care leave. Coordinate maternity leave and related documentation. Support short-term disability, long-term disability, catastrophic leave, and other medical absence cases. Monitor sick absences and coordinate applicable sick-pay approvals. Process and maintain FMLA and Department of Labor-related documentation. Support employees through the return-to-work process following medical leave. Nurse Case Management & Medical Review Review medical records and clinical documentation related to employee absence cases. Evaluate medical information in relation to an employee's job responsibilities. Prepare accurate case notes and medical case summaries. Coordinate with employees and healthcare providers to obtain necessary medical information. Assess whether a fitness-for-duty examination may be appropriate based on available medical information and job requirements. Maintain accurate and confidential case documentation. Communication & Coordination Serve as a primary resource for employees regarding FMLA, medical leave, disability, and return-to-work matters. Collaborate with HR and supervisors regarding employee absences and work restrictions. Communicate with union representatives regarding employee leave and absence matters. Coordinate with healthcare providers and external partners as appropriate. Work with payroll regarding sick-pay approvals and related adjustments. Facilitate communication among employees, HR, management, payroll, healthcare providers, unions, and other stakeholders. Employee Wellness Center Support Support the day-to-day operations of the Employee Wellness Center. Provide clinical support as needed. Respond to medical emergencies at company locations. Support mobile medical operations when required. Monitor and maintain medical supply inventory. Assist with other wellness center activities and assignments. Emergency Response & Travel Participate in the company's emergency response program. Respond to company emergencies outside standard business hours when required. Travel to company locations within the service territory as needed. Support employee wellness operations at various company locations. Required Qualifications Associate Degree in Nursing (ADN) with 4+ years of relevant experience , OR Bachelor of Science in Nursing (BSN) with 2+ years of relevant experience . Current New York State RN license . Experience in nursing, case management, employee health, occupational health, or a related healthcare environment. Working knowledge of FMLA regulations and employee leave requirements . Experience reviewing medical documentation and coordinating healthcare-related cases. Strong written and verbal communication skills. Strong organizational and case management abilities. Proficiency with Microsoft Office and standard computer applications. Valid driver's license. Preferred Qualifications 2+ years of clinical or nurse case management experience . Experience with FMLA and leave of absence administration . Experience with short-term and long-term disability cases. Experience with workers' compensation or disability management. Experience with return-to-work and fitness-for-duty processes . Experience working in occupational or employee health. Experience supporting a unionized employee population. Experience in a corporate Employee Wellness or Occupational Health Center.
