Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluations and treatments based on physician referrals.
• Complete comprehensive evaluations using appropriate tools and procedures.
• Review patient records and establish measurable goals and treatment plans.
• Regularly reassess treatment effectiveness and patient progress.
• Communicate patient needs and progress to the treatment team and relevant parties.
• Consult with medical providers regarding patient evaluations and progress.
• Recommend and assist with adaptive equipment for patient improvements.
• Maintain and submit required documentation according to regulations.
• Supervise Physical Therapist Assistants and Aides in patient care activities.
• Participate in staff development programs and maintain clinical practice skills.

Requirements of the Physical Therapist

• Must have licensure in Physical Therapy
• Experience with Oasis and Home Health is a huge plus
• Proficiency in HCHB software is required
