- Administer medications in accordance with provider orders, medication administration records, Virginia requirements, and community procedures
- Verify resident identity and medication information before administration
- Administer medications at the prescribed dosage and scheduled time
- Observe residents before, during, and after medication administration
- Document medications administered, withheld, omitted, unavailable, or refused
- Promptly report medication errors, adverse reactions, refusals, missed doses, or other concerns to the licensed nurse or appropriate healthcare professional
- Seek direction whenever a medication order, dosage, resident response, or circumstance is unclear
- Monitor residents for changes in physical condition, cognition, behavior, mood, appetite, mobility, or daily functioning
- Promptly report changes or concerns to the licensed nurse or appropriate supervisor
- Obtain and document vital signs or other health information when trained, determined competent, and assigned
- Maintain medication carts, medication records, supplies, and storage areas
- Ensure medications are properly stored, secured, and protected from unauthorized access
- Follow medication-count, controlled-substance, infection-control, resident-identification, and safety procedures
- Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, eating, and personal hygiene
- Assist residents with walking, mobility, positioning, and transfers according to their service plans
- Respond promptly to resident requests and emergency-call systems
- Encourage residents to participate in meals, programs, and community activities
- Provide companionship, reassurance, and respectful personal support
- Document personal care and resident services accurately and promptly
- Maintain resident privacy, dignity, rights, and confidentiality
- Communicate respectfully with residents, families, nurses, providers, pharmacies, and other team members
- Participate in required orientation, medication training, competency evaluations, meetings, continuing education, and in-service training
- Complete additional duties permitted by the employee’s registration, training, demonstrated competency, supervision, and assigned role
- Current and valid Virginia Registered Medication Aide registration
- Registration must be active and in good standing
- Strong attention to detail and commitment to medication safety
- Ability to follow provider orders, pharmacy labels, medication records, community procedures, and supervisory direction
- Ability to recognize and promptly report medication concerns and changes in resident condition
- Strong observation, organization, and time-management skills
- Compassionate, patient, and dependable approach to resident care
- Genuine interest in working with older adults
- Ability to communicate professionally and respectfully with residents, families, nurses, providers, pharmacies, and coworkers
- Ability to communicate clearly and effectively in English, verbally and in writing, as required to understand medication instructions, document administration, report resident changes, and respond during emergencies
- Ability to work independently within the assigned role while recognizing when nursing or provider direction is required
- Previous assisted living, memory care, long-term care, or senior-care experience is preferred
- Previous experience assisting residents with activities of daily living is preferred
- Basic computer skills and the ability to complete electronic documentation
- Dependable attendance and punctuality
- Ability to stand, walk, bend, kneel, reach, and remain active throughout the workday
- Ability to safely lift, transfer, push, or pull up to (physical requirement) pounds with assistance or available equipment
- Ability to successfully complete required background checks, health screenings, training, and employment requirements
- Must maintain an active Virginia Registered Medication Aide registration throughout employment
- Must have completed a Virginia Board of Nursing-approved medication aide training program or otherwise meet Virginia registration requirements
- May only administer medications and perform related duties permitted by Virginia law, the employee’s registration, completed training, demonstrated competency, and assigned role
- Must administer medications according to the prescriber’s instructions for medication, dosage, route, and time
- Must document medication administration accurately in the resident’s record
- Must immediately document and report medication errors and adverse reactions
- Must seek clarification through the appropriate licensed healthcare professional when an order or medication instruction is unclear
- May not independently prescribe medications, change medication orders, select dosages, or make clinical medication decisions
- May not perform nursing assessments, nursing diagnoses, or duties reserved for licensed nurses
- Must complete all required continuing education and population-specific medication training
- Must maintain documentation of required training and competency
- Must wear identification displaying the appropriate Virginia credential while working, as required
- Personal-care duties may only be performed within the employee’s completed training, demonstrated competency, resident service plan, and assigned role
