Type: Full-Time Contract
Location: On-site in Elgin, Texas 78621
Duration: ASAP Through August 31, 2025 (with potential 1 year extension)
Compensation: Pay DOE (Depending on Experience)

This is a vital position focused on improving health outcomes through case management, data reporting, and project leadership.
Key Responsibilities:
- Serve as a Project Team Leader supporting public health initiatives
- Deliver Public Health Case Management services to individuals and families
- Conduct state data reporting and ensure compliance with public health protocols
- Collaborate with local and state health departments
- Provide education, resources, and support to promote community wellness
