Personalized Nursing LIGHT House is a premier behavioral health care provider with 34 years of specialized addiction treatment service serving the adult population. We have locations in Plymouth, Canton, Brighton, & Madison Heights. Specifically, our intent is to increase client well-being, decrease addiction severity, improve the quality of clients' mental and physical heath, and contribute positively to society. All services will integrate substance use disorder, mental health, and primary health care treatment in cohesive and client-centered programs. POSITION SUMMARY We are seeking responsible and compassionate Case Managers to help locate applicable care and resources for our clients and to transport clients to/from approved resource providers. The ideal candidates will take initiative in instituting case management services and communicating with clients in effective ways. Experience with persons with substance use disorder and/or co-occurring disorders is preferred. REQUIREMENTS Bachelor's degree in a related human services field (social work, psychology, criminal justice, etc.). CAADC, CADC or Development Plan from MCBAP. Driver's License and a clean driving record. Applicants must be comfortable driving clients in their personal vehicle. Knowledge of Microsoft Office (Word, Excel, PowerPoint) Be familiar and comfortable with the culture and characteristics of persons with Substance Use Disorders and mental health disorders. Be able to multi-task Be confidential and a good listener Have effective communication skills, both written and verbal PNLH preference for candidates with training in, and experience with, providing trauma related services.
