Salary Range: $223,944 – $280,216 annually (commensurate with experience)

What You'll Do\n

As Site Medical Director, you will provide both clinical leadership and direct patient care.

Clinical Leadership\n

\n
• Lead and supervise physicians, nurse practitioners, and physician assistants at the assigned health center.
\n
• Foster a culture of clinical excellence, accountability, and continuous improvement.
\n
• Monitor provider productivity, quality outcomes, and patient experience metrics.
\n
• Coach and mentor providers through regular performance discussions and professional development.
\n
• Partner with the Chief Medical Officer and interdisciplinary leadership to implement strategic clinical initiatives.
\n
• Support Patient-Centered Medical Home (PCMH), quality improvement, population health, and regulatory initiatives.
\n
• Participate in medical leadership meetings and organizational planning efforts.
\n

Patient Care\n

\n
• Maintain an active primary care practice while serving in a leadership capacity.
\n
• Provide approximately 28 hours of direct patient care and 12 hours of administrative leadership each week.
\n
• Diagnose, treat, and manage acute and chronic medical conditions across diverse patient populations.
\n
• Coordinate care with behavioral health, nursing, pharmacy, and specialty services.
\n
• Participate in after-hours call rotation and maintain hospital privileges as required.
\n
• Support preventive care, chronic disease management, HIV prevention and treatment, gender-affirming care, and whole-person healthcare.
\n

What We're Looking ForRequired Qualifications\n

\n
• MD or DO from an accredited medical school.
\n
• Completion of an accredited Family Medicine residency program.
\n
• Active Illinois Physician License (or ability to obtain).
\n
• Current DEA registration.
\n
• Board Certification (or ability to obtain within two years of hire).
\n
• Minimum three years of clinical practice experience.
\n
• Minimum three years of leadership or supervisory experience, preferably in an ambulatory or community health setting.
\n

Preferred Qualifications\n

• Experience working in a Federally Qualified Health Center (FQHC) or other community-based healthcare organization.
\n
• Experience leading quality improvement initiatives and developing clinical protocols.
\n
• Demonstrated success mentoring and developing providers.
\n
• Commitment to culturally responsive, trauma-informed, and affirming care.
\n
• Experience serving diverse, medically underserved populations.
\n
