Tryfacta is seeking a Licensed Practical Nurse (LPN) for our client in King, WI 54946. This is a temporary contract assignment. Position Title: Licensed Practical Nurse (LPN) Location: N 2665 County Rd QQ, King, WI 54946 Duration: 1 Year Work Schedule Shift: Full time

Shift - Start - End

1st Shift AM - 6:15 AM - 2:45 PM

2nd Shift PM - 2:30 PM - 11:00 PM

3rd Shift NOC - 10:45 PM - 6:45 AM

For all roles: Weekend = Saturday and Sunday. EXCEPTION: Night shift RNs Weekend = Friday and Saturday.

Licensed Practical Nurse (LPN):

• Under the direction of the RN, this is licensed, practical nursing care with responsibility for providing direct and indirect patient care for 50 to 60 geriatric and/or disabled members.
• Shall measure, administer, and record medications as ordered by the physicians.
• Shall perform duties including, but not limited to the following: LPN level nursing procedures and treatments, such as, but not limited to, tube feedings, catheterizations, irrigations, changing sterile dressings, suctioning, tracheostomy care, nebulizer treatments, and specialized skin care treatments to geriatric/disabled members.

Certification & Requirements:

• Updated CV
• LPN Certification
• BLS
• Covid
• 2 Step TB. This would be getting the TB test and a read 48-72 hours later and then another TB test within three weeks of the first one with a read 48-72 hours later. This is an intradermal test. OR QuantiFERON TB Gold OR T-spot blood test
