NURSE PRACTITIONER (CALIFORNIA) - FULL-TIME, EXEMPTSHIFT: 8:30 AM - 5:30 PM PST, Monday-FridayLOCATION: Hybrid (California)MedStreamline is seeking an experienced Nurse Practitioner (NP) to join our growing virtual care team serving residents throughout California. The Nurse Practitioner will play a key role in delivering high-quality, patient-centered care through a combination of telehealth services, remote patient monitoring, chronic care management, and in-person visits as needed.The ideal candidate is a self-motivated clinician with experience in Family Medicine, Internal Medicine, and/or Geriatric Care who thrives in a technology-enabled environment. This provider will serve as a primary point of contact for patients, supporting chronic disease management, wellness initiatives, preventive care, and care coordination while collaborating with physicians and interdisciplinary team members.Applicants must reside in California and maintain a permanent California residence for the duration of employment. Candidates must possess an active California Nurse Practitioner license and hold, or be eligible to obtain, California's 103 NP Certification for independent practice.Essential FunctionsClinical Care• Perform comprehensive health assessments, including medical histories, medication reconciliation, and physical examinations.• Develop and implement individualized treatment and care plans based on patient needs and evidence-based practices.• Evaluate, diagnose, and manage acute and chronic medical conditions.• Order, review, and interpret laboratory, diagnostic, and imaging studies.• Prescribe medications and treatments in accordance with California law and organizational protocols.• Conduct telehealth visits and support remote patient monitoring activities.• Lead care initiation and ongoing management for Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and related programs.• Provide patient and family education regarding disease management, wellness, prevention, and treatment plans.• Collaborate with physicians, nurses, care coordinators, and other healthcare professionals to ensure continuity of care.• Maintain timely and accurate documentation within the electronic medical record system.Care Coordination & Collaboration• Participate in interdisciplinary case reviews and care planning activities.• Coordinate referrals and follow-up care with specialists and healthcare partners.• Communicate effectively with patients, families, providers, and facility partners.• Support quality improvement initiatives and patient outcome measures.• Assist in identifying opportunities to improve clinical workflows and patient engagement.Compliance & Quality• Maintain compliance with California Board of Registered Nursing regulations and all applicable state and federal laws.• Ensure adherence to HIPAA, CMS, and organizational policies and procedures.• Maintain all required licensure, certifications, and continuing education requirements.• Participate in audits, quality reviews, and compliance initiatives as requested.• Promote patient safety, clinical quality, and regulatory compliance.Required• Active, unrestricted California Nurse Practitioner license.• Active California Furnishing Number and DEA registration, if applicable.• Active 103 NP Certification or eligibility and willingness to obtain certification for independent practice.• Minimum of 3 years (or 4,600 hours) of clinical experience in Family Medicine, Internal Medicine, Geriatric Care, or a related primary care specialty.• Board certification as one of the following: Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)• Experience providing patient care independently and within interdisciplinary teams.• Strong assessment, communication, clinical reasoning, and organizational skills.• Proficiency with EMR/EHR systems and healthcare technology platforms.• Ability to work effectively in a remote and hybrid work environment.Preferred• Licensure in additional states.• Experience with Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and value-based care programs.• Experience caring for senior or long-term care populations.• Multi-state telehealth experience.• Experience using population health and remote monitoring technology platforms.Physical Requirements• Prolonged periods of sitting and working on a computer.• Frequent use of phone and video conferencing technology.• Ability to travel throughout assigned service areas as needed.• Ability to perform in-person patient assessments and evaluations.• Ability to lift, carry, and transport equipment weighing up to 15 pounds.• Ability to navigate healthcare facilities, including long-term care and assisted living communities.• Ability to perform the essential functions of the job with or without reasonable accommodation.Success in This Role• Patients receive timely, high-quality, evidence-based care.• Documentation is completed accurately and within established timeframes.• Chronic care and remote monitoring programs achieve strong patient engagement and clinical outcomes.• Patients, providers, and facility partners receive responsive and professional support.• Clinical quality, compliance, and productivity standards are consistently met.• Care plans are effectively coordinated across providers and healthcare teams.• Regulatory, licensing, and documentation requirements remain current and compliant.Why Join MedStreamline LLC?A Better Way for Senior Healthcare MedStreamline is transforming the senior healthcare experience by delivering innovative solutions that improve patient outcomes, reduce unnecessary hospitalizations, and support proactive healthcare management.Focus on People & WellnessOur employees are at the center of everything we do.
