Physical Therapist

Role Overview: A licensed Physical Therapist providing personalized, one-on-one care to older adults in their homes or senior living communities, utilizing a flexible schedule and incorporating telehealth when appropriate. Key Responsibilities Conduct evaluations and develop individualized treatment plans based on evidence-based practices. Deliver therapy sessions aimed at improving mobility, independence, and overall wellness. Document patient progress accurately within the EMR system, adhering to regulatory standards. Collaborate with a multidisciplinary team including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Provide virtual care to enhance access and continuity of treatment. Qualifications & Skills Valid Physical Therapy license and degree from an accredited program. Proficiency in basic computer use and EMR documentation. Current CPR certification. Physical ability to perform patient transfers, repositioning, and other therapeutic activities, including lifting and walking for extended periods.
