Summary

Seeking a dedicated Registered Nurse to support clinical operations and provide high-quality patient care in a community health center. The RN will primarily focus on patient triage while also supporting care coordination, patient education, referrals, and clinic workflow.

Responsibilities

• Conduct walk-in and telephone triage and provide appropriate nurse advice.
• Support clinic flow, scheduling, and provider productivity.
• Assist providers with prescriptions, abnormal test results, and referrals.
• Coordinate follow-up for abnormal tests and diagnostic results.
• Support care coordination for patients seen in the ER or admitted to local hospitals.
• Assist with diagnostic imaging orders and cancer screening follow-up.
• Educate patients on chronic disease management, prevention, risk reduction, and medication adherence.
• Support immunization services and emergency preparedness.
• Assist with audits, reporting, and quality improvement projects.
• Maintain accurate documentation and follow organizational policies.
• Work collaboratively as part of a patient-centered, team-based care model.

Qualifications

• Active California RN license in good standing
• At least 2 years of nursing experience in a community health clinic
• At least 2 years of supervisory experience
• Valid California Driver's License, auto insurance, and clean driving record
• EHR proficiency
• Strong customer service, communication, organizational, and confidentiality skills
• Ability to work independently and within a diverse team
• Bilingual English/Spanish is a plus, not required.
