Fairview Nursing & Rehabilitation Center is seeking an experienced Admissions Coordinator to join our growing team. This role is ideal for an organized, customer-focused professional with prior Skilled Nursing Facility (SNF) admissions experience who enjoys working in a fast-paced healthcare environment.

The Admissions Coordinator will work closely with and report directly to the Admissions Director to ensure a seamless admissions process while providing outstanding customer service to residents, families, hospitals, and referral sources.

• Coordinate the admissions process from referral through admission.

• Verify insurance coverage and obtain necessary authorizations.

• Maintain knowledge of Medicare, Medicaid, Managed Care, and commercial insurance plans.

• Communicate with hospitals, case managers, physicians, families, and referral partners.

• Complete admissions paperwork accurately and efficiently.

• Assist with resident room readiness and coordinate with interdisciplinary departments.

• Maintain accurate documentation and census information.

• Provide exceptional customer service while ensuring a smooth transition for new residents.

• Support the Admissions Director with daily admissions and marketing activities as needed.

• Previous Skilled Nursing Facility (SNF) admissions experience is required.

• Working knowledge of the admissions process in a long-term care or rehabilitation setting.

• Basic understanding of Medicare, Medicaid, Managed Care, and insurance verification.

• Excellent communication, organizational, and customer service skills.

• Ability to multitask and work efficiently in a fast-paced environment.

• Experience with PointClickCare or other electronic medical record systems is preferred.
