RN/LPN

The RN/LPN in a senior living/SNF/LTC setting provides comprehensive, compassionate, and individualized care to residents, ensuring their physical, emotional, and psychosocial needs are met. This role requires clinical expertise, strong communication skills, and the ability to work collaboratively with a multidisciplinary healthcare team Work Setting: Skilled Nursing Facility (SNF) or Long-Term Care (LTC) senior living community providing extended care for residents with chronic illnesses, disabilities, or post-acute recovery needs. Key Responsibilities Assessment & Planning: Evaluate resident health status, develop, coordinate, and implement individualized care plans in collaboration with physicians, therapists, and case managers Clinical Care: Administer medications, perform vital sign checks, monitor medical conditions, and provide therapeutic interventions such as range-of-motion exercises, wound care, and mobility assistance Daily Living Support: Assist residents with activities of daily living (ADLs) including bathing, dressing, eating, and toileting Patient Monitoring: Continuously assess and document resident health status, report changes promptly, and respond to emergencies Education & Support: Provide health education to residents and families, offer emotional support, and promote independence and dignity Team Collaboration: Work closely with physicians, nurses, therapists, social workers, and other staff to ensure safe, coordinated care Documentation: Maintain accurate, timely, and complete medical records in compliance with facility and regulatory standards Environment & Safety: Maintain a clean, safe, and organized work environment, adhering to infection control and safety protocols Required Qualifications License: Active LPN/RN license Experience: Minimum experience in long-term care, skilled nursing, or senior living settings; experience with geriatric care preferred Skills: Strong clinical judgment, patience, empathy, communication, and ability to work in a team-oriented environment Avesta Systems, Inc. is an Equal Opportunity / Affimative Action Employer All qualified applicants will receive consideration for employment without regard to disability, race, color, veteran status, religion, sex, or national origin. #J-18808-Ljbffr Avesta Systems
