Job Description:

The Registered Nurse (RN) Case Manager is responsible for coordinating patient care in the Case Management unit, ensuring efficient use of resources for acute care patients, and facilitating discharge planning. The role requires experience with Meditech and Midas systems.

Responsibilities:

Coordinate patient assessments and develop case management plans.
Facilitate discharge planning for patients transitioning from care units.

Requirements:

Minimum 1-year current acute care Case Manager RN experience.
Active Kansas RN licensure or licensed social worker.
BLS certification.
