Community Health Navigator & Care Coordinator

CareSource is seeking a Community Health Worker to join the inter-disciplinary care team (ICT) to coordinate care for members. The role involves engaging with members across settings, accompanying them to appointments, and coordinating logistics to support care plan goals.
Responsibilities include gathering information for assessments, verifying eligibility and demographics, and contributing to the development and implementation of individualized care plans based on member needs.
