Recovery & Housing Case Manager

Overview The Salvation Army is seeking applicants for a full-time case manager. This position is part of the Joseph McFee Center (JMC), a recovery focused transitional housing program for San Franciscans exiting residential treatment. The JMC program provides participants with the opportunity to build on progress made in early recovery and to work toward a healthy, independent and prosperous life. Under the supervision of the Program Director, the Case Manager’s main goal is to address the needs of JMC participants and cultivate a transformative community based on the program's principles of recovery, accountability, and service. The case manager is responsible for assessing client needs, developing goal plans, providing referral and resource coordination services, client advocacy, crisis intervention, monitoring and evaluating client progress, documenting and reporting client interactions and services, collaborating with staff and partner organizations, empowering and educating clients, and adhering to professional ethics while maintaining client confidentiality. Compensation $28-$31 depending on experience Duties Perform all duties according to policy and procedure and The Salvation Army’s philosophy of care. Manage an assigned case load of up to 25 program participants. Assessment and Intake: conduct interviews and administer assessments to understand the client’s needs, situations, and potential risks. Gather relevant information about the client’s background, family dynamics, living conditions or other relevant factors. Based on the assessment, collaborate with the client to create an individualized action plan that outlines specific goals and objectives as well as interventions that address the client’s unique needs. Establish and maintain relationships with partner organizations and referring agencies. Monitor client progress and reassess their needs to ensure that the goal plan remains relevant and effective. Educate and empower clients by equipping them with the knowledge and skills needed to improve their situation and become more self-sufficient. Respond to and manage crisis situations clients may encounter. Maintain appropriate, professional, and ethical boundaries with staff and clients. Be punctual, flexible, and reliable. Maintain accurate client records on The Salvation Army Social Services Database. Attend all required staff, Corps and Divisional meetings and trainings. Facilitate small group instruction as indicated on the program schedule. Attend community-based meetings as needed. Identify appropriate resources, programs, and services that can benefit the client and make referrals to these services. Coordinate with various service providers to ensure the holistic needs of clients are met. Other reasonably related duties assigned by the Program Director or Program Manager. Assist in the planning, preparation, and execution of special functions such as graduations, holiday celebrations, group activities, or special classes. Required Qualifications High school diploma (College coursework in social services or psychology preferred). Strong verbal and written communication skills. The ability to express empathy and foster a collaborative alliance with clients. Familiarity with San Francisco social services, government resources, community resources, healthcare services, mental health support, and housing services. Excellent organizational skills and ability to work with little direct supervision. Culturally competent and able to work with a highly diverse population. Excellent time management skills. Preferred Qualifications Valid CA Driver License and completion of organization’s driver safety training within introductory period. Experience and ability working successfully with issues of substance abuse, mental illness, and other potential barriers to self-sufficiency. Bilingual in Spanish preferred. Knowledge of co-occurring disorders and trauma-informed environment. Strong electronic communication skills (e.g., Outlook). Proficient in Microsoft Word & Excel. Physical Requirements Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Ability to lift 25 lbs. Ability to operate telephone. Ability to type 35 words per minute.
