Job Summary Registered Nurse (RN) responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients/residents. Collaborates with patients/residents and families, and inter‑disciplinary colleagues, including providers, to plan, implement and evaluate care. Responsibilities Utilize the nursing process (assessment, diagnosis, planning, implementation and evaluation) to provide individualized nursing care. Collaborate with patients/residents, families and providers to plan, implement, and evaluate care. Demonstrate competency within the full scope of nursing expertise and apply appropriate age and population‑specific standards. Provide care for patients/residents in all phases of preventative care, health maintenance, diagnosis, treatment and follow‑up in the home setting. Coordinate care, conduct assessments, educate patients/residents and perform various nursing interventions. Serve as charge nurse when needed and delegate tasks to licensed and unlicensed assistive personnel within scope and competence. Provide assistance and support to delegates to ensure safe, reliable administration of care and resolve care and service concerns. Operate within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules of the state of practice and licensure. Qualifications Graduate from an accredited nursing program (AACN, ACEN, NLN CNEA); Bachelor’s degree preferred. One year of long‑term care, sub‑acute care or medical/surgical experience preferred. Current unencumbered RN license with the State Board of Nursing where practice occurs, and/or multistate licensure under a Nurse Licensure Compact state. Obtain and maintain required certifications, CEUs and competency training. Ability to adapt to age and population‑specific standards. Work Details Shift: Day (United States of America).
