What You'll Do

• Evaluate patients' physical therapy needs and establish personalized goals
• Develop and implement individual therapy plans to improve mobility and independence
• Provide age-appropriate care for pediatric to geriatric patients
• Document all patient care activities accurately and timely in electronic medical records
• Communicate effectively with patients, families, and healthcare staff
• Assess, treat, and monitor patient progress throughout therapy sessions
• Transfer and position patients safely during therapy procedures

What You Need

• Valid Illinois Physical Therapy license (or willingness to obtain)
• Prior experience in physical therapy, preferably in home health or outpatient settings
• Ability to work collaboratively within a healthcare team
• Strong communication and documentation skills

• Experience with diverse patient populations (pediatric to geriatric)
• Knowledge of documentation standards and billing
