PAY: $50.00 - $140.00 per visit Job Type: W-2 Per Diem Medical Specialty: Home Health Work Location: On the road We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills. This opportunity is open for a highly motivated and positive-minded individual. Your Role in our Company Will Be: The RN travels to a patient's home to administer services and helps patients maintain their independence. Duties include: Administering at-home IVs, changing dressings, cleaning wounds, and conducting in-depth assessments. Overseeing patient treatment plans and updating doctors & the agency about patient health. Documenting and communicating with the agency through Synergy Software.

AVAILABILITY:

Work 7 days a week with flexibility. Must be able to work at least two‑three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).

RESPONSIBILITIES:

Providing nurse visits to home health patients (home visits). Performing an initial, comprehensive assessment and uploading documentation to the agency portal. Obtaining and documenting physician orders. In consultation with the assigned Clinical Team Manager, initiate and regularly re‑evaluate and revise the plan of care. Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient's condition and needs. Performing re‑certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager and submitting all related documentation. Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA). Performing Home Health Aide supervisory visits per regulatory requirements. Demonstrating knowledge of regulatory requirements.

POSITION QUALIFICATIONS:
