Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for home health patients
- Implement, monitor, and adjust therapeutic interventions to improve mobility, strength, balance, and functional independence
- Reassess patient progress regularly and modify treatment plans to meet goals
- Document patient care accurately in accordance with professional standards and regulatory requirements
- Collaborate with an interdisciplinary team to coordinate care, discharge planning, and family/caregiver education
- Educate patients and caregivers on home exercise programs, safety training, and fall-prevention strategies

Requirements
- Active involvement in physical therapy practice with the ability to deliver care in a home health setting
- Eligibility to participate in a travel assignment and commit to a 12-week timeframe
- Strong communication and teamwork skills to coordinate with an interdisciplinary group
- Ability to document clinical findings and progress clearly and timely
- Flexibility to adhere to an 8:00 AM–4:30 PM schedule and travel as required
- Prior home health or outpatient PT experience is preferred
