Nurse Practitioner or Physician Assistant - Outpatient Primary Care

This role provides primary healthcare services for non-acute or chronic conditions in specific populations within the CRNP scope of practice, following organizational policies, values, and mission. Work is performed in a physician office or dedicated primary care settings with weekend and on-call coverage as required. Schedule: M-F evenings and Sat 8am-2pm ESSENTIAL RESPONSIBILITIES Provides patient care including recording medical history and physical assessment, developing therapeutic care plans, ordering diagnostics, interpreting results, and accurate medical record documentation. Communicates and coordinates patient care with patient, family, community health services, and interdisciplinary team; involves supervising physician in plan of care and in decisions outside the CRNP scope. Monitors and coordinates patient care with periodic physician evaluations as defined in the CRNP–physician agreement. Prepares written prescription orders, administers medications and injections, sutures wounds, and provides wound care. Educates patients and families on health, preventive medicine, medications, treatment instructions, and appropriate community resources. Triages patient telephone calls and ensures continuity of care. Participates in and supports performance and quality improvement initiatives; ensures regulatory compliance. Performs other duties as assigned or requested to manage outpatient concerns or duties specific to the specialty. Other duties as assigned. QUALIFICATIONS: Required Master's degree in Nursing (MSN) from an accredited program National Board Certification for Nurse Practitioners (AANP, ANCC, NCC, or PNCB) Practicing-state licensure as a Nurse Practitioner Prescriptive Authority from the practicing state (within 60 days of hire) Drug Enforcement Administration licensure (within 60 days of hire) CPR – American Heart Association Act 34 Criminal Background Clearance Act 33 Child Abuse Clearance Act 73 FBI Fingerprinting Clearance Preferred None Required Master’s degree in Physician Assistant Studies Current PA license PA-C or board eligible; ongoing recertification (NCCPA) as PA-C, within 90 days of hire Drug Enforcement Administration licensure (within 60 days of hire) CPR – American Heart Association Act 33 Child Abuse Clearance Act 34 Criminal Background Clearance Act 73 FBI Fingerprinting Clearance Substitutions Four to ten years of related experience and/or training; or equivalent combination of education and experience.
