Job Description

Job Summary:
Provides direct patient care within Long-Term Care or Skilled Nursing Facility settings, supporting healthcare teams through essential nursing assistant duties. Must be adaptable to a variety of shifts, including days, nights, weekends, and holidays, with a focus on delivering quality care.

Responsibilities:

• Perform daily patient assessments to monitor vital signs and overall well-being.
• Assist patients with daily living activities such as bathing, dressing, and mobility.
• Document patient care and observations accurately using EPIC electronic health records.
• Support nursing staff with clinical tasks and ensure compliance with safety standards.
• Maintain a clean and safe environment to promote patient comfort and infection control.
• Communicate effectively with patients, families, and healthcare team members regarding patient needs.

Qualifications:

• Certified Nursing Assistant (CNA) credential required.
• Strong experience with EPIC electronic health record system.
• Ability to work flexible shifts including days, nights, weekends, and holidays.
• Excellent communication and patient care skills.
• Demonstrated ability to follow detailed instructions and document accurately.
• Compassionate, reliable, and adaptable team player.
