Registered Nurse Part-TimeJob Category: Medical/HealthcareRequisition Number: REGIS003253Location: Jamaica Plain, MA 02130, USAJob DetailsDescriptionAre you the RN we have been looking for?JRI's Long-Term Services and Supports (LTSS) program provides care coordination to MassHealth enrollees ages 3-21 with complex LTSS needs as a result of complex medical and behavioral health needs, intellectual and developmental disabilities. Who we are looking for:A part-time (with possibility of increasing to full-time) LTSS RN to provide team oversight of care coordination and clinical care management for MassHealth Members with complex medical and behavioral health needs, who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. The LTSS RN collaborates with their respective Community Partner team and the clinical staff of each Enrollee's ACO/MCO's plan to minimize duplicative efforts, promote integrated care, and ensure quality and continuity of care. The LTSS RN is at the helm of organizing and coordinating resources and services in response to the Enrollee's healthcare needs across multiple settings, and inclusive of both LTSS and Social Determinants of Health (SDoH) needs. This role drives assessment and care planning, care transitions, health and wellness coaching, as well as community and social services connections across a multi-disciplinary care team for CCP enrollees. We want the LTSS RN to have the flexibility needed with a hybrid work model, occasionally in the office and otherwise remote work. We at JRI support our staff, including RN's, in finding the work life balance to meet their professional goals, as well as their personal ones.JRI provides the training so that:You will be able to develop and utilize your clinical skills, managerial skills and overall leadership skills to guide effective trauma-based treatment.You will be able to develop and utilize your leadership skills by providing oversight of medical components of programming for staff and clients to understand their needs more clearly.You will be able to ensure that all staff are working toward a shared mission and vision of the program and provide direct supervision to departmental leaders/Administrators.Bilingual a plus!!
