Requirements
- Home Health experience: Required
- Active BLS certification or willing to obtain: Required
- Valid driver's license and car insurance: Required
- Copy of active nursing license: Required
- OASIS and start of care experience: Strongly preferred
- COVID and FLU vaccination: Required

Roles & Responsibilities
- Prescribe, coordinate, and deliver patient care through the nursing process while maintaining professional nursing standards.
- Supervise Certified Nursing Assistants (CNA) and coordinate care with other healthcare workers as assigned.
- Conduct comprehensive initial assessments and continually assess patients' physical, psychosocial, environmental, and community resources across their lifespan.
- Initiate and modify Plans of Care as needed based on patient evaluations and establish discharge plans in collaboration with patients and healthcare teams.
- Complete OASIS documentation and ensure compliance with federal, state, and agency regulations.
- Assess and address educational needs of patients, families, and caregivers while implementing appropriate teaching strategies.
- Promote a clean and safe environment of care while ensuring patient privacy and confidentiality.
