Oncology Nurse Navigator (FT-Day, 40hrs/week) - Cancer Programs - Cherry Hill, NJ

Job Details The Oncology Nurse Navigator provides expert individualized care coordination to patients, and their families ensuring a timely schedule for diagnostic work-up, results reporting and communication between the patient and the multidisciplinary team ensuring the patient receives necessary referrals and follow-up. The Oncology Nurse Navigator serves as an advocate for the patient, promoting timely, competent, compassionate care and enhances the physician/patient relationship improving the experience of the patient with cancer.
• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Collaborates and continuously partners with chronically ill or high risk patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach.
• Evaluates the needs of cancer patients and educate them about their cancer diagnosis
• Prepares patients and caregivers for what to expect throughout the cancer care continuum.
• Creates and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist and family/caregiver(s).
• Provides clinical support to Patient Navigators and Sidney Kimmel Cancer Center (SKCC) patients.
• Educate patients and families about disease process and potential side effects
• Assist patient with treatment decision making; develop and use decision aids as appropriate
• Attend and support Tumor Boards
• Reduce emergency room utilization and hospital readmissions through outpatient symptom management
• Follow patients throughout the care continuum including inpatient admissions in collaboration with inpatient care management resources
• Support transitions of care and coordination between SKCC sites
• Provide clinical consultation to the Patient Navigators
• Educate multidisciplinary colleagues about the role of the Nurse and Patient Navigators, identified barriers, available resources and the navigation process
• Support program growth and patient experience in all patient interactions

Job Description

Minimum Education and Experience Requirements:
Education:
Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferred.
AND
Experience:
Three (3) years of Healthcare/Medical, experience in Oncology/Cancer Care preferred

Salary Range

$82,243.29 to $117,197.17 Annual

The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.
