Clinical Nurse

The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care based on nursing theory and research. Delivers patient care by incorporating the tenets of a professional practice model of Relationship Centered Care. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the care delivery model and demonstrates effective allocation of resources. Role models relationship-centered care through facilitation of patient interactions and engagement strategies. Considers patient values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and families with concerns and escalates issues as needed. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue and actions. Conducts purposeful rounding on assigned patients to address their needs efficiently. Practices safe and efficient patient-centered care by identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge essential to quality and safety in the patient population served. Implements process improvement strategies based on evidence-based practices and institutional policies. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects. Contributes to quality and safety practices as well as compliance measures. Implements principles of high reliability in patient care to ensure safety. Escalates potential safety hazards or gaps from best practices following established protocols. Utilizes institutional technology by documenting nursing practice to support quality initiatives. Performs charge nurse duties as needed. Coordinates with management to ensure adequate supplies and staffing for patient care. Documents accurately in compliance with regulatory and legal standards of care. Facilitates patient throughput on a shift basis. Contributes to the environment of care to support clinical colleagues, patients, and their families. Advances clinical competence to progress in nursing practice. Projects a professional image and communicates with respect and empathy. Incorporates ethical principles into decision-making for patients and families. Encourages lifelong learning, scholarship, inquiry, and innovation for self and others. Acquires knowledge relative to the role and patient population. Participates in shared decision-making through practice councils and unit initiatives.
