Hospice Transitional Nurse Liaison (RN)

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing job in Troy, Michigan. The Hospice RN will perform comprehensive patient assessments, coordinate start-of-care visits, order DME and medications, and provide education to patients and families. Must have an RN license with associate degree and 2+ years of RN experience; hospice experience preferred.
