Hospice RN Case Manager: At-Home Patient Care & Coordination
St. Gabriel's Hospice and Palliative Care
St. Gabriel's Hospice and Palliative Care is seeking an RN Case Manager to handle admissions and provide nursing care in patients' homes. You will coordinate with the interdisciplinary team and referring agencies, ensuring individualized plans of care and timely paperwork.
Ideal candidates will have 2+ years as an RN in clinical care and 1 year in hospice, with an active RN license and valid driver’s license. Expect on-call duties and field travel as part of routine responsibilities.
