Direct Support Professional I — Weekend Care Specialist
Continuum of Care
Continuum of Care is seeking a Direct Support Professional to assist individuals with intellectual disabilities in their Meriden, CT home. You’ll help with daily living activities, promote independence, and support community involvement with dignity. Duties include hands-on care, documenting in Therap or similar systems, implementing individual plans, and coordinating with the care team.
