In addition to providing relevant direct and appropriate clinical care, treatment and diagnosis to patients within the scope of practice for a primary care physician, the Team Physician functions as a Team Leader in overseeing and directing the full operations of a team-care model, providing whole-person, quality care. Function as a fully accountable colleague with nurse practitioners through collaborative care agreements, and strives to support CHWP’s quality initiatives and requirements necessary and relevant to a Federally Qualified Health Center, and various certification programs, i.e. HRSA, PCMH, Joint Commission, etc.Primary AccountabilitiesAchieve ResultsDirectly affect improvements in patient well-being through accurate and timely diagnosis and delivery of relevant, high quality medical care. Ensure the delivery of competent, accurate, medical care and treatment to all patients as assigned.Provide leadership and direction to all team members and ancillary staff. Affect overall clinic results. Meet or exceed all measurable standards of care, including overall clinical, quality, and productivity goals.Ensure accuracy, efficiency, and appropriate confidentiality with patient charting and related recordkeeping and administrative functions as assigned.Operational ExcellenceEnsure the delivery of all medical treatment and care is consistent with all regulatory, accreditation, and professional standards, including clinical policies and quality initiatives.Understand and ensure all practices and procedures related to the delivery of medical services are consistent with contracted protocols and procedures.Serves as the Team Lead to manage and implement the full operations of the team-care model; engaging all team members input and expertise related to the delivery of whole-person, integrated primary care.Relationship ManagementDevelop and ensure favorable relationships with all patients and their families. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Work may require occasional weekend and/or evening hours. Outpatient call on an equal basis from home.Work hoursFull-time or part-time, exempt salary position with Employee Agreement. Generally, a 50-60 hour work week is required for full-time; part-time as agreed upon by parties; hours are subject to change. Office hours vary by site, but are usually 8:00am to 6:00 pm, Monday through Friday. Some Saturdays and after business hours work may be required. Assigned meetings and travel are a part of the position.WagesStarting wages for physician will be negotiable based on Community Health & Wellness Partnerspay policies and factors such as education and experience, national and state average compensation recommendations based on location and region. Employment compensation and productivity will be described in the Employment Agreement.Introductory PeriodIdentified in the Employment Agreement.Credentialing ProcessPhysician will complete credentialing process with CHWP as to qualification prior to date of hire.
