Clinical Support Specialist

Location This role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within a commuting distance from an office. How You Will Make An Impact Assist Care Managers with administrative discharge planning activities across inpatient and outpatient settings. Monitor and track discharge planning milestones, barriers, and follow-up actions. Procures supplies and equipment in a timely manner to support patient independence in the home. Supports order, delivery, and receipt of Durable Medical Equipment (DME) and medical supplies as identified by patient care team. Facilitates care coordination with external medical providers and specialists. Identifies and schedules referrals with the interdisciplinary care team. Assists patients with scheduling visits with interdisciplinary care team members, as necessary. Assists with and provides non-clinical activities related to HEDIS expectations as determined by quality management leadership. Manages inbound and outbound inquiries and document all correspondences within relevant systems timely. Minimum Requirements Requires a H.S. diploma or equivalent and a minimum of 1 year experience or any combination of education and experience which would provide an equivalent background. Preferred Skills, Capabilities, And Experiences Understanding of Medicaid/Medicare coverages of DME and supplies strongly preferred. Certified Nursing Assistant, Medical Assistant or Licensed Practical Nurse preferred.
