RN FIELD NURSE

Location: Twilight Home Health Care Full‑Time Job Description Primary functions include administering skilled nursing care for patients/clients of all ages in their residence, coordinating care with the interdisciplinary team, patients/clients and their families, and a referring agency. The RN assumes responsibility for and participates in the coordination of care. Responsibilities Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home‑health patients/clients, including OASIS or other assessments as appropriate. Regularly re‑evaluate the patient’s/client’s nursing needs and evaluate outcomes of care. Develop, initiate, and revise the plan of care in partnership with the patient/client, representative (if any), and caregiver(s) to ensure quality and continuity of care. Initiate appropriate preventive and rehabilitative nursing procedures, refer to other services as needed, provide patient/client and caregiver education, and plan for discharge. Furnish services requiring substantial and specialized nursing skill ordered by the physician, counsel the patient/client, caregiver, and family, and use infection‑control measures that protect staff and patients. Coordinate services; inform physicians and other personnel of changes in the patient’s/client’s condition and needs; communicate with all physicians involved in the plan of care; integrate orders from all relevant physicians and ensure they are approved by the responsible physician. Monitor assigned cases to ensure compliance with third‑party payor requirements. Prepare clinical and progress notes; complete appropriate documentation in a timely manner. Demonstrate commitment and professional growth by participating in agency‑sponsored in‑service programs and maintaining or improving competency; participate in the agency’s QAPI program. Supervise, teach, and provide clinical direction to other nursing personnel; assign home‑health aides and supervise LPNs/LVNs and paraprofessionals providing services to patients/clients; may conduct aide competency evaluations if qualified with two years of clinical experience and one year of home‑health experience. Provide effective communication to patients/clients, their family members, team members, and other health‑care professionals. Perform on‑call responsibilities and provide on‑call services to patients/clients and their families as assigned. Qualifications Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing. Current State license as a Registered Nurse or a multi‑state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a NLC state. Current driver’s license with reliable means of transportation. One year of experience as a Registered Nurse in a clinical care setting; home‑health experience preferred. Proof of current CPR certification. Excellent knowledge of nursing care methods and procedures. Excellent knowledge of emergency care. In‑depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.) and willingness to follow them at all times. A team player with excellent communication and interpersonal skills. Responsible and compassionate. Outstanding organizational and multi‑tasking skills. Patient with excellent problem‑solving skills.
