Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year License: Active RN license Certifications: Certified Home Health experience required Must-Have: 1 year of case management experience Description: All applicants must have 1 year of certified home care experience. 1 year of case management experience is required. Experience in hospice, private care, or working as a case manager alone does not meet the qualifications for certified home health care management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Ability to travel to assigned cases is necessary. Weekend rotations occur once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within the organizational philosophy, policy, and standards of community health practice. The role also requires performing admission processes and case management responsibilities in accordance with established policies and standards of practice. Thorough and timely electronic documentation and interdisciplinary communications are essential for this position.
