About the Position
Our client is seeking an experienced Physical Therapist for a Days shift position in Washington, Iowa. This Therapy role is based in the Physical Therapist and requires a minimum of 2 years of recent Physical Therapist experience.
Position Requirements
- Active Iowa license and/or Compact license
- Current BLS and/or ACLS and/or Specialty Certifications
- Minimum 2 years of recent Therapy – Physical Therapist experience
- 2 current clinical references
Documentation Requirements
- Current resume
- Physical exam (within 12 months)
- TB skin test (within 12 months)
- Titers - MMR/Hep B/Varicella
- Respiratory Fit Test (within 12 months)
- Current Tdap and Flu vaccinations
- Ability to pass background check and urine drug screening
