Salary Up to $30.00 / Hour Overview Social Worker – Skilled Nursing Elderwood at Cheektowaga Help residents navigate change, preserve their independence, and remain connected to the people and resources that matter most. Elderwood at Cheektowaga is seeking a compassionate, resourceful Social Worker to support residents and families throughout the skilled nursing experience. In this role, you will address the psychosocial, emotional, and practical needs that influence each resident’s well-being—from admission and adjustment through care planning, family communication, service coordination, and discharge preparation. This is an opportunity to become a trusted advocate for residents while working closely with Nursing, Rehabilitation, Activities, Dietary, medical providers, and other members of the interdisciplinary care team. Responsibilities Your Impact - As our Social Worker, you will help create an environment where residents are treated as individuals, supported in making choices, and encouraged to maintain the greatest possible level of independence and dignity. You will: Assess residents’ psychosocial, emotional, behavioral, and social-support needs Build trusting relationships with residents and families during times of transition, uncertainty, or change Participate in interdisciplinary care planning and advocate for each resident’s preferences, rights, and goals Connect residents and families with internal services, community resources, benefits, and additional support Help residents adjust to the skilled nursing environment while preserving identity, choice, and meaningful relationships Support discharge planning and transitions of care in collaboration with residents, families, and the interdisciplinary team Identify barriers that may affect well-being, treatment participation, recovery, or a successful transition Provide support related to grief, loss, family dynamics, behavioral health needs, and changes in independence Maintain timely, accurate documentation and support compliance with applicable skilled nursing requirements Promote a culture that protects resident dignity, privacy, individuality, and quality of life Qualifications What We’re Looking For - We are seeking a Social Worker who combines strong case-management skills with empathy, sound judgment, and a genuine commitment to supporting older adults and their families. Required Qualifications Bachelor’s degree in Social Work or a related human-services field from an accredited institution; qualifying fields may include sociology, gerontology, psychology, rehabilitation counseling, or special education At least one year of supervised social work experience in a healthcare setting working directly with individuals Previous casework experience Valid driver’s license and a clean driving record Ability and desire to work with older adults and collaborate effectively with residents, families, and interdisciplinary team members Ability to read, write, speak, and understand English at an intermediate or more advanced level to support safe, effective communication with residents, coworkers, visitors, and supervisors Preferred Experience Social work experience involving admissions, discharge planning, transitions of care, or coordination of community services Experience in skilled nursing, long-term care, rehabilitation, or another healthcare environment The right candidate will be an organized, compassionate advocate who can build trust, navigate sensitive situations, and help residents maintain dignity, choice, and the highest possible quality of life.
