Job Details:

Schedule: Monday-Friday | 8:00 AM-4:30 PM | 36 Guaranteed Hours/Week | On-Call/Weekend: No Location: Wilson, NC; Coverage areas include Nash, Edgecombe, and Wilson Counties; Mileage reimbursed between visits per contract company. Job Type: Contract Contract Length: 8 Week/s Pay Range: $52-$54 *Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Manage and coordinate patient care as an RN Case Manager.
• Assess patient needs and develop individualized plans of care.
• Coordinate services and communicate with physicians and interdisciplinary healthcare teams.
• Complete accurate and timely patient documentation and follow-up.

Requirements:

• License(s): Active RN license
• Certification(s): BLS
• Years of experience: 1+
• Skills/Specialties/Technology: Home Health experience preferred; HCHB experience preferred
