Job Title: Home Health RN Case Manager


Location: Baltimore, MD


Pay: $2546.40 weekly


Shifts: Monday to Friday, 8am-5pm


Seven Healthcare are seeking an experienced Home Health RN Case Manager for a 13-week contract in the Baltimore area. This travel assignment involves providing comprehensive case management to patients in Baltimore City, Baltimore County, and potentially Anne Arundel and Howard Counties. The role requires travel between sites to meet patients' needs.

Key Responsibilities of the Home Health RN Case Manager

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement care plans
• Address and reassess cost-efficient, appropriate levels of care
• Coordinate non-medical support for compliance with treatment plans
• Facilitate care transitions and coordinate with healthcare professionals
• Engage specialty resources for optimal health resolutions
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as patient advocate protecting privacy and confidentiality

Requirements of the Home Health RN Case Manager

• Associates or Bachelors Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license in Maryland
• Experience with HomeCare HomeBase and Oasis
• Wound Care experience
• Valid CPR/BLS certification
• Valid Driver's License and Car Insurance
• Ability to analyze complex medical information for decision-making
• Case Management certification
