Physical Therapy Assistant

Pride Health is hiring a Physical Therapy Assistant to support our home health client in Leesburg, FL.

Schedule: 08:00 am to 04:30 pm, Monday-Friday Location: Leesburg, FL Contract Length: 13 weeks Pay Range: $38.00 to $43.00 an Hour *Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Provide home health physical therapy treatments and therapeutic exercises under the supervision of a licensed Physical Therapist, following the established plan of care to improve patient mobility, strength, balance, and functional independence.
• Monitor and document patient progress, response to treatment, and any changes in condition, communicating updates promptly to the supervising Physical Therapist and interdisciplinary care team.
• Educate patients and caregivers on prescribed exercise programs, safety techniques, use of assistive devices, fall prevention, and strategies to support recovery and independence in the home environment.
• Maintain accurate and timely clinical documentation, adhere to agency policies and state regulations, participate in care coordination activities, and support quality patient care across the assigned service area.

Requirements:

• FL Physical Therapist Assistant License.
• Home Care Experience with HCHB experience.
• Type of certification required: BLS
• Minimum Education Requirement: Associate degree.

• 13-week assignments (May be extended based on business needs)
• Work Location: Leesburg, FL
• Coverage Areas: Lake and Sumter Counties
• Will mileage be reimbursed between visits, from first patient to last patient (Y/N)? Y
• On Call/Weekend Requirement: No
• Weekly Hour Guarantee: 32 hours/week
