Provides direct addiction treatment services to adult patients and family members. Essential Duties and Responsibilities Provide individual and group counseling to patients assigned to caseload. Direct training groups and seminars. Provide therapeutic leadership in group counseling sessions. Function as a liaison with other community based agencies and law enforcement agencies to insure continuity of treatment (referrals) and compliance to legal obligations. Complete individual chemical dependency assessment of new patients. Includes completing psychosocial and all required forms. Develop Individual Treatment Plan (ITP) within established time frame of patient’s admission, verified by patient’s signature. Maintain new patient charts current within 30 days. Treatment plans must reflect current problems/goals and all goals expressed “in the words of the person served.” Work with medical staff to secure urine samples for monthly urinalysis. Document results in patient’s chart and program software. Active involvement in Diversion Control through execution of random bottle recalls. Provide necessary forms to the Medical Director for medication requests and other changes. Ensure patient confidentiality by properly completing release of information to all outside referrals and contacts. Maintain patient files in compliance with DHHS regulations and policy. Keep files securely locked. Complete discharge of a patient within seven (7) days of discharge, submit form to Program Director. Contribute to ongoing assessment and treatment planning through daily reports, case reviews, and treatment teams. Provide patient education on chemical dependency and recovery processes. Offer supportive counseling or crisis intervention in conjunction with medical and nursing services as needed. Provide education and public information services to general public and professional community. Participate in professional development, attending internal and external in-service programs and maintaining credentials. Participate in Continuing Quality Improvement and report on assigned CQI monitors. Job Requirements Four‑year degree in counseling or related field, or experience in another field of similar experience, with certification. Acceptable licensure: CAS, LAC, or LPCC. Valid driver’s license or state issued ID. Physical Frequent sitting at desk or computer while typing, completing paperwork, or meeting with patients. Occasional lifting/carrying of papers and files up to 20 lbs. Occasional push/pull exerting up to 5–10 lbs force, opening doors, file drawers. Occasional kneeling/crouching and ability to reach lower file drawers. Mental Position requires constant mental alertness, attention to detail, and high degree of accuracy.
