Overview
Registered Nurse – Home Health, Nashville, Tennessee. This travel assignment starts 06/15/2026 and runs for 13 weeks in a home health care setting. Weekly pay ranges from $2,198 to $2,215 per week with guaranteed 40 hours per week. Shift: 8 hours per day.

Responsibilities
- Administer skilled nursing care for patients of all ages in the home and coordinate with the interdisciplinary team, patient/family, and referring agency
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs
- Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care
- Maintain up-to-date patient records, documenting visits, problems, plans, actions, and goals as they occur
- Educate patients and families on care, medications, safety, and self-care, using developmentally appropriate teaching methods
- Supervise and provide clinical direction to home health aides/LPNs to ensure consistent, quality service
- Assess and reassess pain; apply appropriate pain management techniques and educate on pain control
- Coordinate referrals to appropriate agencies when needs are identified
- Maintain infection control practices and ensure compliance with third-party payer requirements
- Participate in interdisciplinary team meetings and communicate effectively with patients, families, and health professionals
- Perform on-call responsibilities as assigned

Requirements
- Active RN license in Tennessee (current, unencumbered)
- OASIS experience – Required
- Homecare Homebase software experience – Required
- CPR certification – Required
- 1–2 years RN clinical care experience; 2+ years Home Health experience – Required
- Education: ADN or Nursing Diploma program graduate; BSN preferred
- CHPN certification – Preferred
- Current driver’s license and reliable transportation with adequate auto insurance
