Certified Occupational Therapy Assistant (COTA) - Full-Time or Part-Time

Location: Shawano, WI
Facilities: Evergreen Health Services, Shawano Health Services, and Birch Hill Health Services
Setting: Skilled Nursing Facility (SNF)

Continuum Therapy Partners is seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team serving Evergreen Health Services, Shawano Health Services, and Birch Hill Health Services in Shawano, Wisconsin.

This Full-Time or Part-Time Occupational Therapy Assistant opportunity offers the chance to make a meaningful impact while working in supportive, collaborative skilled nursing facilities. As a COTA, you’ll work alongside licensed Occupational Therapists and an interdisciplinary care team to help adult and geriatric patients improve independence, safety, and quality of life.

• Provide occupational therapy treatments under the direction and supervision of a licensed Occupational Therapist.
• Implement individualized treatment plans designed to improve patients’ functional abilities and independence.
• Assist patients with activities of daily living (ADLs), mobility, coordination, strength, and safety awareness.
• Monitor patient progress and communicate updates with the supervising Occupational Therapist.
• Complete accurate and timely documentation in accordance with company, state, and federal guidelines.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members.
• Educate patients and caregivers on therapeutic techniques and safety recommendations.
• Provide compassionate, patient-centered occupational therapy services.

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Wisconsin Certified Occupational Therapy Assistant (COTA) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
