Care Manager: Mental Health Care & Recovery Navigator

Daymark Recovery Services, Inc. is seeking a Care Manager to perform case management, person-centered planning, and documentation under supervision. You will assess client needs, develop care plans, coordinate referrals, and monitor progress to support recovery goals. Responsibilities include crisis intervention, interdisciplinary planning, and ensuring appropriate releases.
