Qualifications

• RN currently licensed in the State of Texas or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
• Valid Driver's License and Auto Insurance
• Demonstrates organizational and leadership abilities.
• Demonstrates a broad base knowledge of community health nursing and competent clinical skills.
• Home Health experience: 1 Year
• Is able to meet physical demand of standing, bending, lifting, stooping or performing other work requiring light physical exertion (up to 30 pounds) on a continuous basis (over 50% of time); or moderate physical exertion (30 to 50 pounds) on a frequent basis (16% to 50% of time); or heavy physical exertion on an occasional basis (up to 15% of time).

Duties

• Directs implementation of approved work methods and procedures that reflect elements essential to rendering high-quality care.
• Ensures all skilled professionals providing care maintain professional standards of community practice.
• Reviews and evaluates existing clinical policies and practices to determine if current methods provide the means for professional staff to carry out their responsibilities and achieve projected goals.
• Coordinates referrals.
• Coordinates patient care.
• Makes patient and personnel assignments.
• Selects and maintains a qualified, well-organized staff to provide care for the needs of patients. Is accountable for all RNs, LVNs, Home Health Aides, and Therapists providing care and services.
• Establishes staffing patterns which reflect the quality and quantity of various professional personnel necessary to plan, provide, and supervise the care rendered to patients and families.
• Assures the development, implementation, and updates of the individualized plan of care.
• Supervises and teaches in order to improve practice within the agency.
• Assures compliance with local, state, and federal regulations.
• Performs other duties as assigned.
• Ensures the client's plan of care/plan of treatment is executed as written.
• Ensures a reassessment of a client's needs is performed by the appropriate healthcare professional.
