Job Summary:

The Speech-Language Pathologist (SLP) provides evaluation and therapeutic services to children, adolescents, adults and seniors. The SLP supports communication, cognitive-communication, voice, fluency, and swallowing needs to enhance functional communication, safety, and meaningful participation in program activities. Services are delivered in a collaborative, person-centered environment consistent with professional, regulatory, and Harbor House standards.

Key Responsibilities:

Evaluation & Treatment:

·        Conduct speech-language pathology evaluations and assessments to identify communication, cognitive-communication, voice, fluency, and swallowing needs in accordance with physician orders and professional standards.

·        Develop, implement, and modify individualized plans of care to address participant-specific communication and functional goals.

·        Provide therapeutic interventions that support functional communication, safety, participation, nutrition, hydration and quality of life.

·        Monitor participant progress toward established goals and adjust treatment approaches as clinically appropriate.

·        Administer standardized and functional assessments as required to support treatment planning and outcome measurement.

Program Support & Therapeutic Services:

• Deliver evidence-based, developmentally appropriate speech-language therapy services across adult and pediatric programs.
• Modify therapeutic activities, strategies, and environments to support participant engagement and functional communication.
• Support communication strategies across program settings to promote carryover of skills.

 

Education & Participant Support:

• Educate participants and families on therapy goals, treatment strategies, cueing and support techniques.
• Provide guidance to staff on strategies that promote effective communication and safe swallowing practices, as applicable.
• Support the use of assistive communication methods, including augmentative and alternative communication (AAC), as appropriate.

Documentation, Quality Assurance & Compliance:

• Complete all required clinical documentation accurately and timely, including evaluations, progress notes, treatment records, and discharge summaries.
• Maintain documentation in compliance with state, federal, payer, and Harbor House requirements.
• Adhere to HIPAA, accreditation standards, infection control policies, and participant rights requirements.
• Participate in required meetings, care planning sessions, and interdisciplinary staffing meetings.

Interdisciplinary Collaboration:

• Collaborate with the Therapy Manager and interdisciplinary team members to support coordinated care and consistent therapeutic approaches.
• Communicate effectively with participants, families, program staff, and external providers to support continuity of care.

Qualifications & Skills:

·        Graduate of an accredited Speech-Language Pathology program at the master’s level.

·        Current, active Speech-Language Pathologist license in the state of Kentucky.

·        Demonstrated ability to provide speech-language pathology services to individuals with intellectual and/or developmental disabilities or comparable populations.

·        Strong clinical judgment, communication, and documentation skills.

·        Ability to work collaboratively within an interdisciplinary team environment.

·        Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) or eligibility and ability to obtain within a specified timeframe, as applicable.

·        CPR/BLS certification or ability to obtain within a specified timeframe.

Physical Requirements:

·        Ability to sit, stand, walk, and move throughout the workday.

·        Ability to engage participants in therapy activities, including positioning and use of therapy materials.

·        Ability to push, pull, and manage therapy equipment as required.

Now Harbor House is expanding to bring these same outstanding day services to children and seniors of all abilities—meaning with and without disabilities—on an intergenerational model where all three groups will engage daily in meaningful opportunities aimed at improving their lives. What does that mean? It means that children, adults, and seniors of all abilities will read, sing, dance, paint, draw, and even exercise together.

We moved into our new 36,000-square-foot Intergenerational Life Center in October 2023, and are now weaving children and seniors into our family, beginning with before- and after-school programs and seniors in 2024, and 105 children of all abilities, ages 6 weeks to 5 years, when our previous 8,000-square-foot building is renovated into a Child Enrichment Center by spring 2025.
