On-Site Clinical Nurse (RN) - Home Care

A. Position Overview

SmithLife Homecare is seeking a compassionate and clinically skilled Registered Nurse (RN) to serve as the On-Site Clinical Nurse for a designated location. This leadership role is responsible for overseeing clinical quality, caregiver supervision, and client care management to ensure safe, high-quality service delivery.

The On-Site Clinical Nurse conducts assessments, develops care plans, supervises caregivers, and supports regulatory compliance while serving as a clinical resource to the care team. This position is primarily based at a single location, with minimal travel required only to support coverage during scheduled PTO or absence of another clinical team member.

This role plays a key part in ensuring SmithLife Homecare maintains high clinical standards, regulatory readiness, and exceptional client care outcomes.

B. Principal Accountabilities

Essential job responsibilities:

Clinical Oversight & Assessments

• Conduct initial nursing assessments for new clients and develop individualized Care Plans within 24 hours of admission
• Perform routine supervisory visits and client reassessments every 90 days
• Conduct supervisory visits every 45 days for clients with complex or skilled needs including high-risk equipment, medication administration, or multiple co-morbidities
• Perform nursing assessments within 24 hours for clients returning home from hospitalization when services resume
• Update Care Plans and clinical documentation when changes in condition occur or when clinically indicated
• Monitor service delivery to ensure care aligns with physician orders, client needs, and agency care standards

Caregiver Supervision, Training & Quality Assurance

• Observe caregiver performance during home visits and evaluate clinical competencies
• Provide training, retraining, and skill reinforcement for CNAs, HHAs, and care partners
• Review caregiver documentation, progress notes, and charting to ensure accuracy and completeness
• Identify documentation or care delivery gaps and implement corrective action plans when needed
• Support clinical training initiatives to maintain caregiver competency and quality care standards

Care Coordination & Clinical Support

• Collaborate with the Administrator regarding client admissions, service planning, and clinical quality
• Communicate with physicians, therapists, and other interdisciplinary team members as needed
• Provide clinical guidance to care coordinators and caregivers regarding client needs or care changes
• Support medication management oversight and skilled nursing needs when required
• Assist with urgent clinical situations and ensure appropriate follow-up care is coordinated

Compliance & Incident Management

• Ensure all clinical documentation meets regulatory requirements and agency standards
• Participate in incident investigations related to client safety, caregiver performance, or changes in condition
• Support abuse, neglect, or exploitation investigations in accordance with regulatory requirements
• Assist with infection control monitoring and public health response when needed
• Prepare and issue discharge notices in compliance with COMAR home care regulations

Team Collaboration & Clinical Leadership

• Serve as a clinical resource for caregivers, care coordinators, and administrative staff
• Promote strong communication between clinical, operational, and scheduling teams
• Participate in interdisciplinary care discussions to support continuity of care
• Contribute to quality improvement initiatives and support clinical process enhancements
• Promote a culture of accountability, professionalism, and compassionate care

D. Required Skills and Experience

Requirements:

• Associate Degree in Nursing required; BSN preferred
• Active, unrestricted Registered Nurse (RN) license in the State of Maryland with ability to be endorsed or currently hold a license in Washington, DC
• Minimum 2–3 years of experience in sub-acute or acute care, home care, or home health healthcare setting
• Strong clinical assessment and care planning skills
• Experience supervising caregivers or direct care staff preferred
• Proficiency with electronic documentation and scheduling systems (e.g., WellSky/ClearCare preferred)
