Registered Nurse - Home Health

Job Description

Job Summary:
Provides comprehensive home health nursing care to patients, utilizing clinical expertise and adhering to regulatory and organizational standards to promote recovery, safety, and quality of life.

Responsibilities:

• Assess patient and caregiver needs through thorough nursing evaluations, ensuring accurate documentation and care planning.
• Develop and communicate individualized, evidence-based care plans that involve patients and caregivers in the decision-making process.
• Serve as Case Manager for assigned patients, managing caseloads and visit schedules to meet productivity standards.
• Complete all necessary OASIS documentation accurately and timely, staying current with evolving requirements and regulations.
• Implement medical plans, nursing interventions, and procedures to ensure safe, effective patient care.
• Collaborate with LPNs and other interdisciplinary team members, delegating tasks appropriately and reviewing visit notes for quality assurance.

Qualifications:

• Current RN licensure in the state, with no restrictions.
• Proven experience with OASIS documentation and home health regulations.
• Demonstrate proficiency in high-tech skills such as IV therapy, wound VAC, and complex wound care.
• Ability to effectively assess patient needs and develop tailored care plans.
• Strong communication skills to coordinate with interdisciplinary teams and document care accurately.
• Knowledge of infection control standards and adherence to federal, state, and organizational policies.
