Work Environment\nAs a Licensed Clinical Social Worker at Nordic Healthcare, you will work remotely, providing essential services via telehealth as well as in-person social work sessions at our Detox Center. Key Responsibilities\n- \n
- Conduct comprehensive psychosocial assessments through in-depth interviews, insightful observations, and thorough reviews of patient medical histories to understand the multifaceted challenges impacting recovery. \n
- Provide empathetic counseling and emotional support to patients and their families, particularly during adjustment periods and challenging emotional experiences associated with recovery. \n
- Collaborate closely with the rehabilitation team to design and implement individualized treatment plans that address the psychological, emotional, and social needs of each patient. \n
- Assist patients in navigating and accessing community resources, including support groups, vocational rehabilitation services, and housing assistance, ensuring smooth transitions into recovery. \n
- Educate patients and their families about available community resources and post-rehabilitation services, equipping them with the knowledge required for continued support. \n
- Engage in proactive discharge planning, working closely with clinical staff to ease transitions back into the home environment or appropriate care settings. \n

- A Master's degree in Social Work from an accredited institution is required. \n
