We are seeking a dedicated Registered Nurse (RN) to provide and coordinate skilled nursing care for patients in their homes.

Position Summary

The RN provides skilled nursing care, completes comprehensive assessments, develops individualized care plans, and coordinates services in accordance with physician orders, Medicare requirements, state regulations, and agency policies.

Responsibilities

• Perform comprehensive and ongoing patient assessments
• Complete Start of Care (SOC), recertification, reassessment, and other required visits
• Complete OASIS assessments accurately and within required timeframes
• Develop, implement, and update individualized plans of care
• Provide skilled nursing services in accordance with physician orders and the plan of care
• Monitor patient condition and identify changes in health or functional status
• Communicate significant changes and patient needs to physicians and the care team
• Coordinate care with physicians, therapists, LPNs, CNAs, and other interdisciplinary team members
• Provide direction and supervision to LPNs and CNAs as applicable
• Complete accurate and timely clinical documentation in the EMR
• Ensure documentation supports medical necessity and regulatory requirements
• Participating in care coordination, team meetings, and required training

Qualifications

• Graduate of an accredited School of Nursing (ASN or BSN)
• Current Florida RN license in good standing
• Minimum 1 year of nursing experience preferred
• Home health, hospice, or acute care experience preferred
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
• Ability to meet all required background and compliance requirements
