The Community Health Worker works to provide outreach opportunities for community residents who lack access to healthcare and health insurance, provide enrollment and application assistance. Employee should maintain a professional image and manner in dealing with patients, families, visitors, and clinic personnel.

Working Conditions/Duties :
Work is partially performed (60%) in a normal, pleasant office environment. In-office work will primarily take place at the Corning clinic location and will focus on insurance enrollment and related tasks. A portion of work time (40%) will be spent performing in-field outreach and enrollment assistance activities. Duties will require travel throughout the health organization's service area. Travel expenses will be reimbursed.
Provide enrollment assistance (including but not limited to assisting consumers with completing coverage applications, gathering required documentation, and troubleshooting the enrollment process) for uninsured children and adults to access subsidized, low-cost, and free health insurance programs through the health insurance marketplace and Medicaid programs
Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness.
Develop, discover, and attend community events in order to promote coverage options and the mission and services of the organization.
Attend and successfully complete all required training programs (as applicable and required by state/federal law) and licensure.
Participate in ongoing conference calls, webinars, network meeting, and other professional development opportunities to maintain expertise in eligibility, enrollment, and program specifications.
Qualifications:
- High School Diploma or GED required.
- Have a valid Driver's License and vehicle.
- Must be able to travel with the use of a personal vehicle. Trave and mileage reimbursement.
- Pass a criminal background check in accordance with State and Federal rules.
- Be able to work weekends
