We’re seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Community Transitional Recovery Program (CTRP) team.

This is an opportunity to use your nursing skills in a recovery-focused behavioral health setting where your work directly supports individuals as they transition from institutional or acute crisis settings toward greater stability and independence.

As an LPN, you’ll provide hands-on nursing care, health monitoring, medication support, and care coordination while collaborating with participants, families, program staff, and healthcare providers.

About Community Transitional Recovery Program

The Community Transitional Recovery Program (CTRP) of Resources for Human Development (RHD) provides short-term assessment and recovery supports to individuals with high and complex needs who are transitioning from institutional or acute crisis settings back into the community.

CTRP serves as an important building block toward independence. The program helps participants stabilize, develop essential skills, strengthen their health and wellness, and prepare for successful community living.

Our goal is to empower individuals to make choices about their recovery and live as independently as possible in the least restrictive community setting appropriate to their needs.

What You’ll Do

As a Licensed Practical Nurse , your primary role will be to provide comprehensive oversight, coordination, and hands-on nursing care for health-related needs affecting program participants.

You'll address both direct and indirect health concerns through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers.

Your Responsibilities May Include:

Nursing Care & Health Monitoring

• Assess participants' physical and emotional health needs.
• Provide direct nursing care and interventions based on individual needs.
• Monitor ongoing medical conditions and changes in health status.
• Recognize health concerns requiring additional intervention or medical attention.
• Communicate significant health concerns to appropriate clinical and program leadership.
• Promote preventive healthcare, wellness, and healthy living practices.

Medication Management

• Support safe and effective medication management.
• Administer medications and treatments according to physician orders and program procedures.
• Maintain accurate medication documentation and records.
• Monitor participants for medication effectiveness, side effects, and other concerns.
• Educate participants about medications and health-related needs.
• Communicate medication concerns to appropriate healthcare professionals.

Care Coordination

• Collaborate with physicians, nurses, clinical professionals, families, and program staff.
• Help coordinate medical appointments and healthcare services.
• Follow up on healthcare recommendations and treatment needs.
• Communicate changes in participant health conditions to appropriate members of the care team.
• Help ensure continuity of care between outside healthcare providers and CTRP staff.
• Advocate for participants' healthcare needs.

Recovery & Independent Living Support

• Encourage participants to take an active role in managing their health and wellness.
• Provide health education that supports greater independence.
• Help participants develop skills needed to successfully manage health needs in the community.
• Support individualized recovery and transition goals.
• Promote choice, dignity, self-determination, and independence.

Documentation & Team Collaboration

• Maintain accurate and timely nursing documentation.
• Maintain medical and medication records according to program requirements.
• Participate in interdisciplinary team meetings and care coordination activities.
• Maintain participant confidentiality and follow HIPAA requirements.
• Attend required trainings and meetings.
• Collaborate with program staff to maintain a safe and responsive environment.
• Perform additional duties as assigned.

Qualifications

Required

• Current and valid Licensed Practical Nurse (LPN) license
• Strong clinical assessment and nursing skills
• Knowledge of medication administration and management
• Strong written and verbal communication skills
• Ability to maintain accurate nursing and medical documentation
• Ability to maintain confidentiality and professional standards
• Ability to work collaboratively with an interdisciplinary treatment team
• Ability to complete required employment screenings and training

Schedule & Compensation

• Part-Time
• $31.00/hour
• Community Transitional Recovery Program (CTRP)
• Schedule flexibility may be required based on participant and program needs.

Physical Requirements

• Ability to sit, stand, walk, bend, reach, and move throughout program and community environments.
• Ability to assist participants as needed while performing nursing responsibilities.
• Ability to perform essential job functions with or without reasonable accommodation.
