Job Profile Summary This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders. The primary responsibilities include assessing, planning, implementing, and evaluating the care needs of a designated group of patients, providing nursing care and identifying clinical support tasks within the scope of practice for the achievement of the patient’s plan of care as directed. The position requires a nursing license and focuses on advanced knowledge of job areas typically obtained through advanced education and work experience. The incumbent may direct the work of lower level professionals, manage processes and programs, coach and review the work of others, resolve complex problems, and work independently with limited supervision. Job Overview The professional registered nurse possesses and applies the skills and knowledge necessary for the delivery and management of patient care from admission through discharge. The nurse is knowledgeable of current trends in healthcare, advocates for patients and families, and facilitates a collaborative environment among all health care providers. Responsibilities include planning, delivery, and management of patient/family centered care utilizing the nursing process and adhering to standards of nursing practice embedded in research, evidence-based practice, and best practices. The nurse at Tufts Medical Center provides clinical excellence that fosters an environment central to our patient and family centered model of care. Minimum Qualifications Registered Nurse (RN) license. Basic Life Support (BLS) certification. Certifications (e.g., ACLS, PALS) relevant to the department and type of care being provided may be required. Preferred Qualifications Bachelor of Science in Nursing (BSN). Specialty Nursing certifications. Duties and Responsibilities Utilizes theoretical and evidence-based knowledge in partnering with patients and their families to assess, plan, implement and evaluate age appropriate care. Assesses and collaborates with patient, family/significant others and other disciplines to identify expected outcomes based on synthesis of data and diagnosis; and to develop a care plan that enhances the healthcare experience. Plans and develops individualized outcomes-based care plans, orchestrates and validates a plan of care for each shift, initiates interdisciplinary patient/family meetings, communicates and coordinates the patient’s discharge plan, and ensures the plan utilizes principles and concepts of project, organizational or systems management. Implements the plan safely and timely, documents implementation and any modifications, collaborates with nursing colleagues and others, utilizes evidence-based interventions and treatments, provides and coordinates health teaching, and documents per hospital and regulatory requirements. Evaluates the effectiveness of the planned strategies in relation to patient responses and the attainment of expected outcomes, and updates the plan of care as needed. Demonstrates skills and judgement necessary to implement nursing interventions and procedures, utilizes evidence-based practice, incorporates new knowledge and strategies to initiate change in nursing care practices, and provides culturally and ethnically sensitive care. Performs procedures and treatments using nursing knowledge and judgment, administers and assesses medication response, maintains clinical competency with respect to technology, serves as an advocate and educator for patients and families, and provides high quality care efficiently and safely. Participates in patient safety and quality activities, uses principles to support and improve patient outcomes, participates in performance improvement activities through data collection, analysis, and evaluation, and champions quality, safety, regulatory, and patient-family satisfaction. Facilitates cooperative relationships among disciplines, supports collaboration to implement the plan of care, delegates responsibilities to others, and influences others to achieve service excellence by establishing a climate for optimal patient care, promoting a safe environment, and identifying customer service needs. Participates in professional development and lifelong learning activities, maintains current licensure, seeks educational opportunities, mentors newly hired staff and nursing students, promotes evidence-based practice, and advances professional practice through shared governance. May assume charge nurse functions in collaboration with unit leadership. Facilitates communication by seeking and sharing job-related information, assuming responsibility for professional conduct, and actively participating in staff meetings and unit communications. Physical Requirements Prolonged, extensive, or considerable standing/walking. Lifts, positions, pushes, and/or transfers patients and equipment. Considerable reaching, stooping, bending, kneeling, and crouching. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise, and possible exposure to radiation, lasers, electric shock, etc. Regular exposure to the risk of bloodborne diseases and other transmissible infections. Contact with patients under a wide variety of circumstances. Subject to varying and unpredictable situations. Handle emergency and crisis situations. Subjected to irregular hours. May have contact with hazardous materials. Skills & Abilities Possesses and applies skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages, and cultural patterns. Provides care for a designated group of patients according to unit guidelines and hospital policies, procedures, and protocols. Has skills and knowledge to provide care to the age groups of the population served, including knowledge of growth and development, ability to interpret age-specific data and responses to care, and ability to provide age-appropriate communication.
